To Create a New Form Dialog Containing All Items

Choose Settings > Forms.
In the Standard Form Items dialog, turn all items on, and click OK.
Create a new form as described in Building Simple SELECT Statements.

Note: The Query text field is present because Query Multiline was turned on in the Standard Form Items dialog.
