Title Case ignores words that consist of all upper case letters. To change all upper case text to Title Case, first convert to Lower Case and then to Title Case.

First Capital capitalizes the first letter of the first word of the selected text and ignores all other characters. To change text with arbitrary capitalization to First Capital, first convert to Lower Case and then to First Capital.
